What to Expect
Registration:
- Please arrive 15 minutes prior to your scheduled appointment time to complete the registration process. Please bring:
- Your child’s insurance card
- Your child’s social security number
- Completed health history forms (which may be downloaded from this site)
- Any forms or questionnaires that have been mailed to you prior to the appointment
- Insurance and demographic information is verified at every visit.
Evaluation:
- The evaluation will take approximately one hour and will include:
- A review of the health and developmental history
- A review of previous medical records, testing and reports
- A review of questionnaires
- A complete neurological and comprehensive exam
- Additional questionnaires and screening tools might be used
- Additional studies such as lab work or medical imaging may be ordered
- A referral to another specialist (i.e., neuropsychologist, neurosurgeon, psychologist, occupational, speech or physical therapist) may be made to help the neurologist better diagnose and treat the nature of your child’s problem
- Follow-up care will be scheduled
Follow-Up Care:
- The neurologist will recommend a follow-up appointment after all testing is completed.
- Please schedule the follow-up visit when leaving.
