What to Expect

Registration:

  • Please arrive 15 minutes prior to your scheduled appointment time to complete the registration process. Please bring:
  • Your child’s insurance card
  • Your child’s social security number
  • Completed health history forms (which may be downloaded from this site)
  • Any forms or questionnaires that have been mailed to you prior to the appointment
  • Insurance and demographic information is verified at every visit.


Evaluation:

  • The evaluation will take approximately one hour and will include:
  • A review of the health and developmental history
  • A review of previous medical records, testing and reports
  • A review of questionnaires
  • A complete neurological and comprehensive exam
  • Additional questionnaires and screening tools might be used
  • Additional studies such as lab work or medical imaging may be ordered
  • A referral to another specialist (i.e., neuropsychologist, neurosurgeon, psychologist, occupational, speech or physical therapist) may be made to help the neurologist better diagnose and treat the nature of your child’s problem
  • Follow-up care will be scheduled


Follow-Up Care:

  • The neurologist will recommend a follow-up appointment after all testing is completed.
  • Please schedule the follow-up visit when leaving.
