What to Expect

Registration:

Please arrive 15 minutes prior to your scheduled appointment time to complete the registration process. Please bring:
Your child’s insurance card
Your child’s social security number
Completed health history forms (which may be downloaded from this site)
Any forms or questionnaires that have been mailed to you prior to the appointment
Insurance and demographic information is verified at every visit.

Evaluation:

The evaluation will take approximately one hour and will include:
A review of the health and developmental history
A review of previous medical records, testing and reports
A review of questionnaires
A complete neurological and comprehensive exam
Additional questionnaires and screening tools might be used
Additional studies such as lab work or medical imaging may be ordered
A referral to another specialist (i.e., neuropsychologist, neurosurgeon, psychologist, occupational, speech or physical therapist) may be made to help the neurologist better diagnose and treat the nature of your child’s problem
Follow-up care will be scheduled

Follow-Up Care:

The neurologist will recommend a follow-up appointment after all testing is completed.
Please schedule the follow-up visit when leaving.
